As prescribed in Clause 3.1 of Article 29; Item b of Clause 1.3 of Article 34 and Section I of Annex 3, promulgated and enclosed with Decision No.959/QD-BHXH dated September 09, 2015 of the General Director of the Vietnam Social Insurance to promulgate provisions on collection management of social insurance, unemployment insurance; management of social insurance books, health insurance cards, in case the employee applies to renew social insurance book by changing family name, name, middle name; date of birth; gender, ethnicity, nationality, dossier elements include:

- The social insurance and health insurance participant’ declaration which provides and changes information (Form TK1-TS);

- Social insurance book;

- Birth registration certificate;

Client can complete the dossiers and then submit to the social insurance agencies to be considered and resolved.
