Dossier for enjoyment of the voluntary retirement regime guidance includes:

1. Social insurance books.

2. An application form for retirement regime of persons covered by voluntary social insurance (Form 12-HSB) or pending pension entitlement certificate for monthly allowances under Decree No. 09/1998/ND-CP for the employee’s leave, waiting about the conditions on retirement ages for the regime (in case of loss of a certificate of pending entitlement they must submit their application form of 12-HSB).

3. A written explanation of the reason for a late filing case after the time for pension enjoyment. The explanation content shall clearly state the person job, his/her residence and commit to take responsibility before the law for the contents of explanation.

In cases filling late because of the imprisonment duration or illegal exit and legal entry or to be declared missing returns by the Courts, he/she shall fill Certificate of Completion of Sentence (copy or certified copy) or early amnesty paper (certified copy or copy) the Court’s decision of declaration missing cancel (certified copy or copy) or decisions, documents of competent agencies to permit to reside in Vietnam (certified copy or copy) enclosed with proof of the time entry (certified copy or photocopy).

4.  The written record of assessment of the working capacity decreases,  made by the Medical Assessment Council, for employees who retire (the original).

Note: In case of submitting copies, please bring the original for comparison.

Dossier for settlement of a voluntary lump-sum social insurance allowance:

1. Social insurance books.

2. An application for enjoyment of lump-sum social insurance allowance (Form 14-HSB).

Dossier for enjoyment of the voluntary survivorship allowance regime:

1. Social insurance books.

2. Death certificate (certified copy or photocopy) or death notice (certified copy or photocopy) or the Court’s decision to declare a death (certified copy or photocopy).

3.  Declaration of dead person’s relatives (Form 09A-HSB).

Note: In case of submitting copies, please bring the original for comparison.
