1.                  Conditions of dental clinic

1.1. . The infrastructure:

a) Fixed position, separate from the family's living place, ensuring adequate light, with dust-proof ceilings, walls and floors must use easy-to-clean and hygienic materials.

b) Specialized clinics must have clinics and medical treatment with an area of ​​at least 10 m2 and places for patient reception (except for health consultation clinics or health counseling rooms through information technology means) ). , telecommunication). Particularly for specialized surgery rooms, specialized cosmetic clinics, there must be patient rooms with an area of ​​at least 12 m2; rehabilitation clinics must have more rehabilitation rooms with an area of ​​at least 10 m2; HIV / AIDS consultation and treatment clinics must have an area of ​​at least 18 m2 (excluding the waiting area for examination), divided into two rooms to perform the function of medical examination and counseling for patients.

c) In addition to the above provisions, depending on the scope of registered professional operations, the maxillofacial clinic must meet the following additional conditions:

- In case of performing the procedure, including implant technique, the procedure room must have an area of at least 10 m2;

- In case of a dental - maxillofacial - dental clinic having more than 01 dental chair, the area for each dental chair must be at least 5 m2;

- In case a specialized clinic uses radiation equipment (including dental X-ray equipment attached to dental chairs), it must meet the provisions of law on radiation safety;

d) A separate sterilization area must be arranged to handle medical equipment used reuse.

d) Satisfying the conditions of radiation safety, medical waste disposal, fire prevention and fighting according to law provisions; Ensure sterility for the procedure room.

f) Having enough electricity and water and other conditions for service and carepatient.

1.2. Medical equipment:

 

a) Having sufficient medical equipment and instruments suitable to the registered practice scope;

b) Having anti-shock first aid kits and sufficient specialized emergency drugs;

c) Health consultation clinics or health counseling rooms through information technology and telecommunications facilities which are not required to have medical equipment and instruments specified at Points a and b of this Clause but must fully meet the following conditions: information technology means, telecommunications and equipment suitable to the scope of operation registration.

1.3. Personnel:

a) Any chief physician of a specialized clinic must meet the following conditions:

- Being a doctor with a practice certificate suitable to the specialty registered by the clinic.

- Having provided medical examination and treatment for at least 54 months in that specialty.

In addition, particularly for dentistry specialized dentistry clinics, the person in charge of professional and technical matters must meet the following conditions:

 Cosmetic clinic: A plastic surgeon or plastic surgery specialist;

b) Apart from persons in charge of professional and technical activities of specialized clinics, other subjects working in specialized clinics, if having conducted medical examination and treatment, must have practice certificates and be assigned to them. work in accordance with the scope of professional practice stated in the practitioner's practice certificate.

2. Application for registration of a dental clinic

- An application for an operating license, made according to a set form

- A valid copy of the establishment decision or a document with the name of the medical examination and treatment facility, issued by a competent state agency, for a medical examination and treatment facility, or a certificate of enterprise registration for private medical examination and treatment establishments or investment certificates for foreign-invested medical examination and treatment establishments;

- A valid copy of the practicing certificate of the person in charge of professional and technical activities of the medical examination and treatment facility; persons in charge of specialized sections of medical examination and treatment establishments;

- A list of registered practitioners at medical examination and treatment establishments (including registration of practitioners and medical practitioners at the establishments but not required to have practice certificates) according to the set form

- Copy declaration of facilities, medical equipment, organization and personnel of medical examination and treatment facilities according to the Form

- Documents proving that the medical facility satisfies conditions on facilities, medical equipment and personnel organization suitable to the scope of professional activities of one of the prescribed organizational forms. in Section 1, Chapter III of Decree 109/2016 / ND-CP;

- The list of technical and professional qualifications of the proposed medical examination and treatment facility, based on the list of technical expertise issued by the Minister of Health;
